GUFS Student Online Regstration

Please begin your student’s registration by completing the online registration portion (directions in sections below). Please make sure that all copies of forms and documents are clear and readable so that we can electronically accept them.

For kindergarten registration, please scroll down.

  • If this is your first child entering Garrison School, click this link: https://esdparentportal.lhric.org/Garrison
  • This will bring you to the Garrison UFSD Parent Portal screen.
  • There is no need to log in. Simply click “Online Registration” below the log-in fields. (You may also access Parent Portal by scrolling to the bottom of the District website and selecting the “Parent Portal” link.)
  • Parents will receive a Parent Portal account after their child(ren) is enrolled in Kindergarten, before the start of the school year.
  • Then click Online Registration, as shown in the screen shot below:

If you already have children in GUFSD and you have a Parent Portal account, log in to Parent Portal.

  • Then click Register New Student (top right), as shown below:

  • Once you have completed and submitted the form, you will receive an email confirmation.
  • You will also receive an email directly from Operoo, our document management system, with a User ID and Password, along with instructions to log in and complete the additional information required for your child’s Kindergarten registration.
  • Your registration is not complete, and students cannot be scheduled, until this documentation has been received by the District.
Change of address

If your address changes it is essential that you provide the district with updated information. Please complete the form below and submit it, along with proof of residency at the new address, to Central Registration. Central Registration will notify individual school(s) of the change.

If a bus change is needed, the transportation department will be notified.

We look forward to having your student join our community of learners and creative thinkers. Please use this step-by-step guide to assist you through the online registration process.

Who should Register

Please complete the online registration steps below if your child is:
  • Registering for the Universal Pre-K Program:
    • Pre-K Registration opens: February 15, 2023
    • Students must turn 4 years of age by December 1st of the school year enrolled in. Students must also be toilet trained by the start of the program. There will be 18 placements for the Pre-K program and a lottery system will be used should the registration counts exceed this number.
  • Registering as a Kindergarten Student enrolling for the first time in Public School:
    • Kindergarten Registration opens: February 15, 2023
    • Students must turn 5 years of age by December 1st of the school year enrolled in. Students must also be toilet trained by the first day of school.
  • Registering as a Pre-K through 8th grade Transfer Student transferring from a different school district to the GUFS District
  • Registering for purposes of a CPSE / CSE Service

If none of the above applies to you and you fall into one of the situations below, please review what is required:

High-School Aged Children: You will complete the online registration with us for purposes of providing any transportation needs. You will need to provide proof of residency to our office so that we may send a letter to your high school to confirm your residency (or continued residency if you are switching between high schools affiliated with our district).

Out-Of-District Tuition Paying Families: If your address is not within the GUFS district but you wish to enroll under the tuition paying status out of district, this is an option. Please contact Joe Jimick at 845-424-3689 x552 for more information.

Registration Steps

  1. Complete the Online Enrollment
  2. Operoo Forms - As part of the enrollment process, you will be sent a welcome letter from Operoo to provide additional required documents and supplemental information. Once you receive your welcome letter, please log in, and complete all forms that pertain to each student you are registering.

Operoo Forms / Document Requests

A listing of all the registration document requests/forms you may see in your Operoo Account are provided below. This listing is intended to help you organize any supplemental information you may be asked for in Operoo.

  • Certified Copy of Child’s Birth Certificate; or a Passport

Digital versions of the forms below, as they pertain to your child’s enrollment, will be available in your Operoo Account online. 

  • Affidavit of Legal Responsibility: All applicants are to complete this affidavit
  • Parent/Guardian Identification: This form keeps a form of identification on file for those authorized to pick up the student.
  • Affidavit of Surrendering Legal Responsibility: If applicable, complete this affidavit (and the one below) if the student resides with a relative/friend where the custodial parent resides out of district. This form states all rights to custody of the child are relinquished (used in lieu of court documents).; And
  • Affidavit of Person(s) in Parental Relation: If applicable, complete this affidavit if a Parent Affidavit Regarding Student Residency is filed.  This form is completed by the custodial guardian of the student.

Digital versions of the forms, as they pertain to your child’s enrollment, will be available in your Operoo Account online.

Home Language Questionnaire: Required

Current Physical Examination & Immunization Records: Required
To be completed by a student's health practitioner with an official stamp or signature of a healthcare professional. This must be conducted no more than 12 months prior to and submitted no more than 30 calendar days after the student’s entrance into school.

Health Information Form: Required
Completed by the Parents/Guardians.

Dental Health Certificate: Optional
This is an optional form to be completed by the student's dentist.

Divorce/Separation Custody Papers/Adoption or Foster Care Papers (if applicable)
You (the parent, guardian, or caregiver) have to show that the youth is living with you and that you have total and permanent custody and control.

Adoption or Foster Care Papers (if applicable)

Educational Records from Previous School
(your child’s previous school will send these items, if applicable)

  • Report Card
  • State Test Results
  • IEP Plan – Special Education
  • Section 504 Plan

Potassium Iodide Form: Optional
This is a form that GUFS is mandated to offer you but your participation is optional. It will be found within your Parent Portal once your child is registered.

Release of Records Form: Required for any students transferring from another school district into GUFSD or between High Schools affiliated with the GUFSD.

Housing Questionnaire (if applicable)
